Kidney Care
Description of Services: Help to slow the progress of kidney disease and prevent associated health problems that occur when kidneys are not working well

The patient care team includes: nephrologists, renal registered nurses, renal social workers, renal pharmacists, renal dietitians and a recreation therapist

Services include:
  • explain treatment options if kidney disease does progress and provide guidance on the type of dialysis that best fits a patient's lifestyle. Many options available
  • offer psychosocial support and palliative care - offered to patients with End Stage Kidney Disease (ESRD) who choose not to have a renal replacement therapies or kidney transplant
  • explore pre-emptive transplant options (kidney transplant before going on dialysis) are explored with the patient as appropriate

Fees: Most services covered by OHIP * fees for some services
Application: Referrals to this program will be made by Osler nephrologists
