Description of Services: Patients are referred to the clinic for assessment of a variety of concerns such as:
  • recent changes in physical, mental or functional abilities; falls, impaired mobility, dementia, delirium, change in mood, decreased ability for self-care;
  • major changes in support needs, caregiver stress, institutional placement being considered; and
  • safety concerns – physical, psychological, social, and environmental; or a sudden increase in the use of health care services over the last 6 months

Eligibility - Population(s) Served: Seniors 65 years and older
Application: Referral by a physician is required * the clinic will accept patient referrals from Emergency department, post discharged and the community * will consider other referrals that do not meet age criteria
