Eligibility - Population(s) Served: Seniors who have one or more of the following: live alone, difficulty walking, functional decline, frequent falls, 5 or more medications, history of cognitive impairment, suspected elder abuse or neglect, ED use in previous 30 days, as well as an RN or any other emergency's team member professional recommendation
